Some fees associated with a psychological or neuropsychological evaluation can be covered by health insurance, depending on the individual's policy and the specific services provided.

Hazel Approach Psych Services is in network with Aetna, Blue Cross Blue Shield, and Cigna. The cost of qualifying services, based on medical necessity are eligible to be paid by the insurance company.
Self-Pay & Out of Network
There are several reasons why someone might choose to pay out of pocket to cover psychological services, even if they have health insurance or other coverage options. Some of these reasons may include:
Confidentiality: Some individuals may prefer to pay out of pocket for medical or psychological services to maintain confidentiality. If they use their health insurance, certain information about their diagnosis and treatment may be included in their medical records, which could be accessed by their employer or insurance company. By paying out of pocket, they can avoid having this information shared with others.
Choice of provider: Some individuals may prefer to see a provider who is not included in their insurance network or who does not accept insurance at all. Paying out of pocket allows them to choose the provider they prefer, rather than being limited to those in their insurance network.

Comprehensive Psychological Evaluation & Behavioral Risk Assessment
- The fee for a comprehensive evaluation (e.g., juvenile risk assessment, trauma evaluation, or parenting capacity evaluation) includes services that are either not covered by insurance or only partially covered by insurance given the breadth of services performed.
- An itemized estimate of cost will be provided at the onset of services, following an initial consultation.
- The rate and total estimated cost will be determined based upon information gathered during the initial consult. Only a portion of services might be covered by your insurance.
75 Minute Consultation for Individuals Fee: The fee for consultation is only applied if the consultation appointment is distinctly separate from an evaluation. The current rate is $300 per hour.
Organizational Consultation: Estimated cost is based upon need; rates negotiable
